Job Coach - (YEAR ROUND POSITION Supported Employment): (M-F - 800a - 1:00p, no benefits)
Position Requirements:
* Valid and clean NYS Driver's License (pre-hire check) is mandatory
* Successful completion of Pre-Hire criminal background check required
* 1+ years experience working with the developmentally disabled population
* High School Diploma or Equivalency required
* Ability to perform physical tasks and work alongside the consumers
Position Responsibilities:
*Find out job assignment for the day from Supervisor
*Find out clients assigned to you by the supervisor for the day and any pertinent information needed regarding the consumers that have been assigned to you
*Transport clients to and from remote work sites using Agency vehicle
*Provide direct supervision and orientation to the consumers while working at remote work sites
*Ensure all required job tasks are completed satisfactorily *Serve as a positive role model to the consumers and model appropriate work behavior
*Communicate all client behaviors and issues to the SEMP Supervisor and the assigned Case Manager
*Complete required "job specific" paperwork
*Attend weekly staff meetings as required *Maintain communication with SEMP Supervisor while at remote work sites and advise of any issues/concerns *Communicate overall site performance to the SEMP team
*Be aware of and implement documented consumer goals and objectives
*Follow and adhere to all Agency and program specific policies and procedures
*Perform other duties as assigned
